THE SIMPLE GUIDE FOR MEDICAL CARE AND DECISION-MAKING
INSTRUCTIONS FOR PRINTING AND FOLDING:
1) This brochure is a 4-panel, 8 1/2" x 14" brochure, which will need to be printed on two sides onto LEGAL-SIZED, 8 by 14 paper. (The print will be too small and the columns won't align if it is printed on regular 8 by 11 sized paper.)
2) Copy the second side on to the back of the first side (first side has photo and title of brochure).
3) Folding instructions. With the first side of the brochure (showing the photo), fold the paper in half along the right side of the front panel, then fold again along the left side of the front panel, so that the front of the brochure is on top.
